Terros Health seeks a Full-Time Health Navigator at our 27th Ave location in Phoenix to help patients access care and navigate services within FQHCs. You will focus on MOUD program support, health education, and resource linkage, delivering patient-centered guidance.

Ideal candidates have a high school diploma or GED with behavioral health experience; associate’s or bachelor’s preferred. Bilingual skills and AZ driver’s license are valued. Flexible schedule and comprehensive benefits are offered.

Qualifications

• High School Diploma or GED with 1 year of behavioral health experience.
• Associate’s or Bachelor’s degree preferred in health education or related field.
• 1.5 years of behavioral health experience preferred; bilingual skills a plus.

Responsabilités

• Educate and advocate for patients, addressing barriers to care.
• Coordinate appointments, referrals, and follow-up across primary and behavioral health services.
• Connect patients to community resources and social determinants of health supports.
• Support MOUD program navigation and care plan adherence.
