Job summary

Terros Health is seeking a Full-Time Health Navigator to support patients in navigating healthcare services within FQHCs, with a focus on the MOUD program and closing care gaps in the Phoenix Metro Area.

The role emphasizes education, access to primary and behavioral health services, and collaboration with care teams to improve outcomes for underserved populations. Valid Arizona driver’s license is required.

Qualifications

• High School Diploma or GED required.
• Minimum one year experience in care coordination, outreach, navigation, social services, healthcare, or related field.
• Knowledge of community resources and social determinants of health.
• Basic life support (BLS) required.

Responsibilities

• Provide outreach, engagement, care coordination, and patient navigation services.
• Conduct outreach in shelters, schools, and community events.
• Build trusting relationships to improve healthcare engagement.
• Assist patients in accessing primary care, behavioral health, and social services.
• Report barriers to quality care to supervisors.
• Coordinate referrals and track follow-up.
• Document encounters in the EMR on the day of service.
• Support transitions of care after ED visits or hospitalizations.
• Conduct Social Determinants of Health screenings and assist with housing, transportation, and other support services.
