Qualifications

• High School Diploma or GED + 1 year behavioral health experience required.

Responsibilities

• Provide outreach, engagement, care coordination, and patient navigation within Mobile Health programs.
• Conduct outreach in shelters, schools, and community settings to engage underserved populations.
• Build trusting relationships to improve healthcare engagement and adherence to care plans.
• Assist patients in accessing primary care, behavioral health, and substance use treatment.
