Job description

Seeking a Health & Mental Health Coordinator to oversee the management, planning, implementation, and evaluation of comprehensive health, mental health, dental, and nutrition services for children and their families enrolled in the Early Head Start Home-based program.

This position ensures children and families receive necessary medical, dental, and mental health services, maintains up-to-date health records, and performs routine health-related and clerical tasks to support the program's health and nutritional needs.

Position Highlights

1. Complete health histories at enrollment, vision, hearing screenings within 45 days, and other health services as needed.
2. Ensure children are current on preventive and primary health care, working with home visiting staff to meet follow-up needs.
3. Provide training on health, dental, nutrition, and mental health topics to staff, families, and pregnant women.
4. Gather documentation for referrals to Early Intervention or Chicago Public Schools for children with suspected disabilities and support families through the process.
5. Assist in negotiating services at schools or with community agencies, attend staffing meetings, advocate for children, and support families in accessing community resources.
6. Ensure parent consent and release of information are obtained and on file for all health services.
7. Support other projects as directed by the Early Learning Academy Director of Family Services.
