Job summary

The Providence Community Health Centers, Inc. is seeking a Health Information Associate to support care teams by analyzing and responding to patient health information requests.

You will sort, review, and scan medical reports into the electronic health record, maintain complete health records, and ensure confidentiality. Responsibilities include tracking records, validating patient identifiers, assisting with releases, and supporting lab reconciliations and patient inquiries while maintaining

Qualifications

• Knowledge of Rhode Island state and federal laws/regulations and HIPAA/privacy regulations.
• Ability to read/write/speak English.
• Medical terminology knowledge is required.
• Proficient computer skills with Windows-based applications.
• Experience with Electronic Health Records (EHR) preferred.

Responsibilities

• Electronically track new patient health records for appointments and triage.
• Use the medical records portal to locate patient status and charts for clinical staff.
• Scan and file medical reports into the EHR per policies.
• Verify patient name and DOB before scanning into charts.
• Review and sort authorizations and ensure HIPAA compliance.
• Assist with uploading digital faxes and lab reconciliations.
• Support patients with release forms and accessing protected health information.
