Job summary

Lexington Regional Health Center is seeking a Health Information Management Clerk to support chart preparation, scanning into the electronic medical record, and maintaining orderly records at the Lexington campus.

The role includes reviewing records for completeness, handling requests for protected health information in compliance with HIPAA, data entry for birth and death certificates, and collaborating with the Health Information Management team to ensure accuracy and patient safety.

Qualifications

• High school diploma or equivalent.
• At least one year of experience in health information management preferred.
• Medical terminology knowledge preferred.
• Ability to maintain confidentiality and handle sensitive information.

Responsibilities

• Prepare charts for scanning and scan charts into electronic medical record system, sorting charts by documents and placing them in chronological order.
• Review medical records for completeness; ensure all documentation is completed and fields signed; address electronic deficiencies.
• Respond to requests for copies of PHI per HIPAA; analyze authorizations for HIPAA compliance.
• Answer main office telephone.
• Collect and enter data for birth certificates.
• Collect and enter data for death certificates.
• Collaborate with other Health Information Management employees.
• Abstract information from medical records for chart review.
• Monitor deficiencies and notify visiting specialists for completion.
• Ensure patients and visitors follow infection control guidelines.
• Ensure environment meets governing body standards.
• Handle duties that may involve exposure to bloodborne pathogens.
• Maintain patient and staff safety using safety tools.
• Regular attendance at the assigned work location.
• Perform other duties as assigned.
