Job summary

Terros Health is seeking a Health and Housing Specialist in Tempe, AZ to deliver direct, community-based housing support and behavioral health services. The role emphasizes coordination with medical providers and community resources, with a focus on housing readiness and tenancy support.

Expect a full-time schedule with mileage reimbursement, bilingual differential pay, and a robust benefits package including 401(k) and generous PTO. SMI experience is strongly preferred for the position.

Qualifications

• Requires High School Diploma/GED with BH experience or an BH-related degree path.
• SMI experience strongly preferred as a plus.
• Customer service and engagement with clients are essential.

Responsibilities

• Provide direct face-to-face community-based services to help clients obtain and maintain housing.
• Conduct screenings, intakes, and assessments to evaluate needs and develop treatment goals.
• Engage clients into PSH services and orient them to the H20 program.
• Provide transportation to clients as needed using a personal vehicle or Terros Health vehicle.
• Coordinate care with families, behavioral and medical providers, and community resources.
• Assist clients with connection to physical health services and follow-up.
