Job summary

Terros Health is seeking a Health and Housing Specialist to support clients in Phoenix, AZ. The role focuses on helping clients obtain and maintain housing and coordinating care across health and behavioral health services.

The position is full-time with 40 hours weekly, involving 80% field work and 20% office work. Bilingual pay differential and comprehensive benefits are offered to qualified candidates.

Qualifications

• High School Diploma/GED with 1.5 years of behavioral health experience or higher education in related fields.
• Bachelor’s or higher in behavioral health; related experience preferred.
• Interpersonal, communication and collaboration skills; professional relationships.

Responsibilities

• Provides direct face to face community-based services to assist clients to obtain and maintain their housing.
• Conducts screenings, intakes, and assessments to determine needs and develop treatment goals.
• Provides outreach to engage clients into PSH services and orients to the H20 program.
• Provides transportation to clients as needed using agency vehicle or own vehicle when available.
• Coordinates care with families, clinical and medical providers, and community resources.
• Assists clients with connection to physical health services and primary care follow-up.
• Delivers excellent customer service to clients and partners as needed.
