Job summary

Terros Health seeks a Health and Housing Specialist in Phoenix to deliver community-based housing services within an interdisciplinary clinical team. You will work face-to-face with adults including SMI, substance use, and dual-diagnosis individuals to secure and maintain housing.

The role requires 80% fieldwork and 20% office time, with a 40-hour workweek and flexible shifts. Extensive benefits, PTO, and differential pay enhance the position.

Qualifications

• High School Diploma or GED with 1.5+ years of behavioral health experience, or an BH-related degree.
• Valid Arizona driver’s license and 3+ years driving experience.
• Must pass drug screen, TB test, background check and fingerprint clearance.
• Excellent interpersonal and communication skills with teams and clients.

Responsibilities

• Provide direct face-to-face community-based services to assist clients in obtaining and maintaining housing.
• Conduct screenings, intakes, and assessments to determine needs and goals.
• Coordinate care with family, medical providers, and community resources.
• Provide transportation to clients as needed using personal or agency vehicle.
