Job summary
Terros Health is seeking a Health and Housing Specialist for the Priest Drive Recovery Health Center in Tempe, AZ. This full-time role combines community-based housing support with BH services, requiring travel up to 80% with mileage reimbursement.
Typical schedule is Monday–Friday, 40 hours weekly, with two shift options. The role emphasizes housing assistance, care coordination, and collaboration with medical providers to support client stability.
Qualifications
- High School Diploma/GED with BH experience or related degrees.
- Associate’s in medical/behavioral health; or bachelor’s with BH experience.
- Bachelor’s in BH or related field may substitute BH experience.
- SMI Experience strongly preferred.
Responsibilities
- Provide direct face-to-face community-based housing support services.
- Conduct screenings, intake, and assessments to determine client needs and goals.
- Coordinate care with client’s family, health providers, and community resources.
- Assist clients with connection to physical health services and primary care follow-up.
- Deliver excellent customer service to clients, providers, and stakeholders.
