Job summary

HeartShare is seeking an Health Home Outreach Coordinator to develop and maintain referral partnerships that drive enrollment growth for the Adult Health Home and Children's Health Home programs.

You will conduct community-based outreach, build relationships with healthcare systems, and engage prospective members, guiding them through enrollment while maintaining accurate logs and reports.

Qualifications

• High school diploma or GED required; Associate degree preferred in human services, social services, healthcare, business, communications, or related field.
• Experience in community outreach, care management, healthcare, behavioral health, social services, enrollment, recruitment, or relationship development preferred.

Responsibilities

• Collaborate with Health Home leadership to develop and maintain referral partnerships for AHH and CHH enrollment growth.
• Develop relationships with hospitals, health systems, MCOs, providers, schools, community organizations, shelters, foster care and preventive programs.
• Conduct proactive outreach through field visits, events, presentations, calls, and electronic communications.
• Explain Health Home services and eligibility to prospective members and support enrollment processes.
• Maintain an active outreach pipeline and follow up with leads and partners.
• Track outreach activities, referral sources, lead status, enrollment progress, and outcomes.
• Deliver presentations to internal and external stakeholders about Health Home services and processes.
• Identify barriers to referral and enrollment and work with staff to resolve them.
• Monitor outreach trends and adjust strategies with program leadership.
• Maintain knowledge of AHH and CHH eligibility criteria and enrollment processes.
• Provide accurate referral logs, enrollment information, and status reports.
