HeartShare is seeking a Health Home Outreach Coordinator in New York to build and maintain referral partnerships and support enrollment growth for our Adult Health Home and Children's Health Home programs.

The role conducts community-based outreach, engages prospective members, and collaborates with hospitals, health systems, and social service partners to streamline eligibility and enrollment while delivering accurate outreach reports.

Qualifications

• High school diploma or GED is required; associate degree preferred.
• Experience in community outreach or enrollment and relationship development is preferred.
• Strong relationship-building and partner management skills.
• Ability to meet outreach, enrollment, and performance targets.

Responsabilités

• Develop and maintain referral partnerships to grow AHH and CHH enrollment.
• Engage healthcare systems, hospitals, clinics, schools, and community organizations.
• Conduct proactive outreach via field visits, events, and presentations.
• Explain Health Home services and eligibility to prospective members.
• Maintain active referral pipeline and accurate records of outreach activities.
• Deliver presentations to stakeholders about Health Home services and enrollment processes.
• Identify and resolve barriers to referral and enrollment in a client-centered manner.
• Prepare weekly/monthly status reports on outreach and enrollment progress.
