Job summary

Community Health Action of Staten Island (CHASI) seeks a Health Home Coordinator to lead the Health Home team, supervise care managers, and ensure high-quality services for clients with chronic illness. The role focuses on engagement, documentation review, and maintaining program standards.

Qualifications

• Three years of direct service experience in Health Home or chronic disease management.
• Bachelor’s degree required; Master’s degree may be considered in lieu of some experience.
• At least 1 year of supervisory or management experience.

Responsibilities

• Provide direct supervision to Care Managers for effective interventions and performance.
• Track client engagement and program performance.
• Ensure coverage of caseloads and perform case management as needed.
• Review and approve client documentation per program standards.
• Oversee eligibility processes and enrollment for HCBS services.
• Offer crisis intervention to distressed clients.
• Prepare data reports to monitor compliance and indicators.
