Job summary

TAG MedStaffing is seeking a Health Home Care Coordinator for an on-site role in Brooklyn, NY. You will outreach to engage eligible individuals, coordinate access to healthcare, behavioral health, and social services, and help enroll in Medicaid and other programs.

Responsibilities include needs assessments, field visits, documentation, and ongoing client support to achieve health and social outcomes. A high school diploma and NY driver's license are required, with 2+ years in care coordination

Qualifications

• High school diploma or GED required.
• Valid New York State driver's license with clean driving record.
• Minimum 2 years of care coordination or related experience.
• Experience with chronic health or behavioral health services preferred.
• Proficiency in Microsoft Office and electronic databases.

Responsibilities

• Engage eligible individuals via community, phone, and electronic outreach to enroll in services.
• Coordinate access to healthcare, behavioral health, social services, and community resources.
• Perform initial needs assessments and field visits to identify client needs and barriers.
• Assist with Medicaid and benefit applications and entitlements.
• Monitor progress toward goals and address barriers to care.
• Maintain accurate client records and required reports in electronic systems.
