Job Overview – Health Home Care Coordinator

Compensation: $26/hour
Location: Brooklyn, NY
Schedule: Monday to Friday (On-Site)

TAG MedStaffing is hiring a Health Home Care Coordinator in Brooklyn, NY for our client, supporting care coordination, community outreach, and resource navigation for individuals with complex healthcare and social service needs. This role will work directly with clients in community and office settings to connect them with healthcare, benefits, and supportive services. The position requires regular field-based work throughout New York City.

Responsibilities as the Health Home Care Coordinator:

• Client Outreach: Conduct community, telephone, and electronic outreach to engage eligible individuals and coordinate enrollment in available services.
• Care Coordination: Connect clients with healthcare, behavioral health, social services, benefits, and other community-based resources.
• Needs Assessment: Complete initial assessments and field visits to identify client needs, barriers, and appropriate support services.
• Benefits Navigation: Assist clients with applications for Medicaid and other eligible benefits, entitlements, and community programs.
• Client Support: Monitor progress toward individualized goals and help clients address barriers to maintaining healthcare and social services.
• Documentation: Maintain accurate client records, progress notes, service activity, and required program reporting within electronic systems.

Qualifications for the Health Home Care Coordinator:

• Education: High school diploma or GED is required.
• Licensure: Valid New York State driver's license with an acceptable driving record is required.
• Experience: 2+ years of care coordination, case management, community outreach, social services, or related experience is required.
• Industry Knowledge: Experience working with individuals with chronic health, behavioral health, or complex social service needs and knowledge of community resources is required.
• Technical Skills: Proficiency with Microsoft Office, electronic databases, internet-based systems, and client documentation is required.
• Skills & Attributes: Strong communication, client advocacy, organization, problem-solving, confidentiality, and independent fieldwork skills are required.
