The role includes collaboration with the care team, supporting discharge planning, and ensuring timely documentation. A High School diploma is required; a Bachelor's in Health or Human Services is preferred.

Qualificações

• High School diploma or GED is required.
• Bachelor’s degree in Health or Human Services related field preferred.
• At least 2 years of work-related experience.

Responsabilidades

• Collaborates with the interdisciplinary care team to develop and coordinate the care plan.
• Directs field activity with the Patient Navigator to ensure information flow across the team.
• Identifies barriers to care and helps patients overcome them; supports discharge planning and care transitions.
