Job summary

Crusader Community Health in Rockford, IL is seeking an Outreach and Enrollment Navigator to educate residents about the Illinois Health Insurance Exchange, the Marketplace, and affordability programs such as Medicaid and CHIP. The role guides community members toward choosing coverage that fits their needs, providing information and support.

The position reports to the Central Scheduling Manager and requires a High School diploma with preference for healthcare or social services experience;

Qualifications

• Education: High School diploma or equivalent.
• Experience in healthcare, social services, or education preferred.
• Proficiency with Microsoft Office and basic computer skills.
• Ability to work in a fast-paced healthcare setting.

Responsibilities

• Educate consumers about the Illinois Health Insurance Exchange and Marketplace.
• Explain insurance affordability programs such as Medicaid and CHIP.
• Assist individuals in enrollment and documentation collection.
• Coordinate with scheduling and care teams to facilitate access to services.
