The Cadence Health team seeks a Health Coach Supervisor to join our Care Delivery team. You will play a dual role: delivering expert health coaching to patients while also leading and supporting a team of Health Coaches. You will oversee daily operations, ensuring that best practices in coaching, education, and patient engagement are upheld. You will also be responsible for fostering a collaborative, high-performing team culture that prioritizes growth, development, and excellence in patient care.

WHAT YOU’LL DO

  • Conduct virtual coaching sessions with patients, utilizing motivational interviewing, SMART goals, and evidence-based interventions to drive sustainable health improvements.
  • Provide ongoing mentorship, coaching, and professional development for your team, ensuring they have the skills and resources needed to excel in their roles.
  • Lead bimonthly team meetings focused on best practices, patient challenges, and strategic improvements in care delivery.
  • Monitor and analyze key performance metrics, identifying trends and implementing strategies to optimize patient outcomes and team performance.
  • Develop and refine training materials, coaching strategies, and learning tools to support continuous team development.
  • Ensure all quality standards and compliance requirements are met through regular audits and data-driven assessments.
  • Collaborate with leadership to communicate policies, procedures, and best practices, ensuring consistency in patient care and service delivery.
  • Assist with recruiting, interviewing, and onboarding new team members, fostering a strong and cohesive team environment.
  • Work flexibly across time zones to provide coverage and support for your team as needed.

WHAT YOU’LL NEED:

  • Active health coaching certification (NBHWC, NSHC, ACE Advanced).
  • 5+ years of experience in health coaching, patient education, or chronic disease management.
  • 2+ years of experience in a leadership or supervisory role, including mentoring, training, and team development.
  • Strong background in behavior change techniques, patient engagement, and chronic disease coaching.
  • Proven ability to foster a positive team culture, motivate employees, and drive performance improvement.
  • Experience in developing training programs, coaching materials, and professional learning tools.
  • Excellent communication and leadership skills, with the ability to manage change initiatives effectively.
  • Strong analytical mindset, with the ability to interpret data, identify trends, and implement data-driven strategies for improvement.
  • Experience in a virtual healthcare setting with remote patient monitoring technology is a plus.
