Job summary

CountryHouse Elkhorn, an Agemark Senior Living community, is seeking a Health & Wellness Director to lead the wellness team and deliver compassionate, person-centered care to residents.

You’ll develop individualized service plans, oversee medications and documentation, and collaborate with families, physicians, and care partners. Nebraska RN or LPN license required with at least 1 year of senior care and 2+ years of leadership experience.

Qualifications

• Active nursing license (RN or LPN) in Nebraska.
• At least 1 year senior care experience.
• At least 2 years leadership/supervisory experience.
• Strong clinical documentation and care planning knowledge.
• Ability to work evenings or weekends; Manager on Duty when needed.

Responsibilities

• Lead and mentor wellness team; ensure high standards of resident care.
• Develop individualized service plans based on assessments and orders.
• Coordinate care with residents, families, physicians, and providers.
• Ensure documentation, medication management, and compliance with state/company standards.
• Perform resident assessments and adjust care needs.
• Oversee safe medication practices and staff education.

At CountryHouse Elkhorn, an Agemark Senior Living community, we’re looking for an experienced Health & Wellness Director to lead our wellness team and ensure residents receive compassionate, person-centered care. What You’ll Do

• Lead the Wellness Team: Guide, mentor, train, and support team members while promoting high standards of resident care.

• Oversee Resident Care: Develop and maintain individualized service plans based on resident needs, assessments, and physician orders.

• Build Strong Partnerships: Communicate with residents, families, team members, physicians, and healthcare providers to coordinate care.

• Maintain Compliance: Ensure documentation, resident records, medication management, and wellness practices meet state requirements and company standards.

• Assess Resident Needs: Complete assessments for current and prospective residents and identify changes in care needs.

• Support Medication Management: Oversee safe and accurate medication practices and provide education and guidance to the team.

• Manage Risk & Emergencies: Identify residents at risk, develop proactive care plans, and respond appropriately to emergencies.

• Stay Hands-On: Remain visible and engaged throughout the community, providing direct support when needed and ensuring quality care.

What You’ll Bring

• Active Nursing License: Current RN or LPN license in Nebraska required.

• Senior Care Experience: At least one year of experience working with seniors and a strong understanding of their health and wellness needs.

• Leadership Experience: At least two years of experience leading, supervising, or developing a team.

• Clinical Knowledge: Strong understanding of medications, resident assessments, care planning, and clinical documentation.

• Strong Leadership Skills: Excellent communication, organization, problem-solving, and relationship-building abilities.

• Compassionate Approach: A commitment to resident dignity, independence, and living our EPIC ACTS values every day.

• Flexibility: Ability to work evenings or weekends and serve as Manager on Duty as needed.
