CountryHouse Elkhorn, an Agemark Senior Living community, seeks an experienced Health & Wellness Director to lead our wellness team and ensure compassionate, person-centered care for residents in our Nebraska community.

You will oversee care plans, medication management, compliance, and staff development while staying hands-on to uphold high standards and support families, physicians, and staff.

Qualifications

• Active Nursing License required (RN or LPN) in Nebraska.
• Senior care experience and leadership background preferred.
• Strong clinical documentation and care planning skills.

Responsabilités

• Lead the Wellness Team, mentoring and supporting staff while upholding high resident-care standards.
• Oversee individualized service plans based on assessments and physician orders.
• Coordinate with families, physicians, and providers to ensure coordinated care.
• Ensure documentation, medication management, and compliance meet state and company standards.
• Perform resident assessments and identify changes in care needs.
• Manage safe medication practices and provide team education.
• Identify risks, develop proactive care plans, and respond to emergencies.
• Maintain visibility in the community and provide hands-on support as needed.

At CountryHouse Elkhorn, an Agemark Senior Living community, we’re looking for an experienced Health & Wellness Director to lead our wellness team and ensure residents receive compassionate, person-centered care. What You’ll Do

• Lead the Wellness Team: Guide, mentor, train, and support team members while promoting high standards of resident care.

• Oversee Resident Care: Develop and maintain individualized service plans based on resident needs, assessments, and physician orders.

• Build Strong Partnerships: Communicate with residents, families, team members, physicians, and healthcare providers to coordinate care.

• Maintain Compliance: Ensure documentation, resident records, medication management, and wellness practices meet state requirements and company standards.

• Assess Resident Needs: Complete assessments for current and prospective residents and identify changes in care needs.

• Support Medication Management: Oversee safe and accurate medication practices and provide education and guidance to the team.

• Manage Risk & Emergencies: Identify residents at risk, develop proactive care plans, and respond appropriately to emergencies.

• Stay Hands-On: Remain visible and engaged throughout the community, providing direct support when needed and ensuring quality care.

What You’ll Bring

• Active Nursing License: Current RN or LPN license in Nebraska required.

• Senior Care Experience: At least one year of experience working with seniors and a strong understanding of their health and wellness needs.

• Leadership Experience: At least two years of experience leading, supervising, or developing a team.

• Clinical Knowledge: Strong understanding of medications, resident assessments, care planning, and clinical documentation.

• Strong Leadership Skills: Excellent communication, organization, problem-solving, and relationship-building abilities.

• Compassionate Approach: A commitment to resident dignity, independence, and living our EPIC ACTS values every day.

• Flexibility: Ability to work evenings or weekends and serve as Manager on Duty as needed.
