Job summary

NCCF seeks a Healthcare Services Navigator to coordinate referrals to healthcare providers and community resources for Black residents in Montgomery County. The goal is to enhance access to providers within one business day, documenting utilization and satisfaction across a wide range of health resources.

The role requires intake, health assessments, and assisting clients with health insurance applications, followed by connecting to providers and scheduling appointments, with follow-up on

Qualifications

• AA or BA degree and two years related experience or equivalent.

Responsibilities

• Engage individuals who request referral to a health provider, support their access to professionals and navigate the health care system.
• Provide one-on-one navigation to connect individuals to culturally tailored health networks and essential community services.
• Complete intake and health assessments and assist with needs based on results.
• Ensure clients apply for and/or acquire health insurance before connecting to a health professional.
• Help individuals complete the health insurance application process.
• Connect clients to health professionals who accept their coverage and help with appointments.
• Provide follow-up regarding appointments and assess client satisfaction.
• Enter client information into the BPHN data management system promptly, including appointments and follow-ups.
• Provide appointment reminders and follow up.
• Engage community organizations and build partnerships with County resources as referral sources.
• Participate in BPHN events as invited vendor.
• Perform miscellaneous duties as assigned.
