Visiting Nurse Association Community Services is seeking a Personal Navigator to connect residents with public benefits and enrollment services across Montgomery County communities. The role emphasizes compassionate outreach, data-driven case management, and coordination with PENNIE, COMPASS, and Medicare.gov to improve access to health coverage.

Qualifications

• Undergraduate degree in social work, psychology, public health or related field preferred; case management experience required; master's preferred.
• Excellent verbal and written communication; engage diverse community members.
• Empathy for community needs and compassionate approach.
• Strong problem-solving and creative solutions for vulnerable populations.
• Cultural competence and sensitivity to diversity.
• Organizational and time management; ability to work independently or in a team.
• Strong networking to connect with community leaders and resources.
• Computer proficiency and use of digital tools for communication and data collection.
• Familiarity with Montgomery County human services and local resources.
• Social justice interest and advocacy experience.
• Public benefits experience: PENNIE, CHIP, Medical Assistance, Medicare; knowledge of eligibility; awareness of TANF, SNAP, etc.
• Bilingual Spanish with immigration status experience preferred.

Responsabilités

• Provide screening, intake, advocacy, enrollment, renewal, and/or referral assistance to residents seeking public benefits.
• Assist enrollment through PENNIE Health Insurance Marketplace, COMPASS or Medicare.gov for insured and uninsured individuals.
• Distribute outreach materials and represent the agency at community events and outreach activities.
• Build relationships with local organizations and networks to promote the program.
• Track data for case management, reporting, and grants, including client counts and renewal dates.
