JOB DESCRIPTION:

The Health Access & Affordability Project Coordinator supports UVM Health initiatives across Vermont and Northern New York designed to reduce financial, transportation, geographic, digital, and other nonclinical barriers to care.

Under the direction of the Community Health Improvement Manager, the Coordinator organizes project activities, facilitates collaboration among UVM Health partner hospitals, clinical and operational teams, and community organizations, maintains timelines and deliverables, and tracks progress and outcomes.

The position gathers input from patients, community members, partner hospitals, and regional partners to help identify recurring access challenges and inform practical, locally responsive improvement efforts.

Areas of focus may include cost of care, financial assistance, medication affordability, transportation, rural and digital access, and health system navigation.

The coordinator also supports Community Health Needs Assessment and Community Health Improvement Plan priorities across UVM Health's service area.

The role focuses on project coordination and systems improvement rather than ongoing case management or direct-service navigation.

The position reports to the Community Health Improvement Manager and works in strategic alignment with the System Director of Health Equity and Community Health.

EDUCATION:

Bachelor's degree in public health, community health, social work, human services, health care administration, public administration, or a related field required. An equivalent combination of education and relevant experience may be considered.

EXPERIENCE:
  • Two to four years of relevant experience in project coordination, community health, public health, human services, health care access, rural health, care navigation, or a related field preferred.
  • Experience supporting projects involving multiple departments, community organizations, or other external partners preferred.
  • Experience working with communities or populations experiencing barriers to health care access beneficial.
  • Experience with community engagement, data tracking, quality improvement, or grant-funded projects preferred.
