Job summary

Buckeye Hospice and Palliative Care Services in Ohio seeks a Clinical Services Coordinator/Director of Clinical Services to lead the hospice clinical program. You will supervise clinical staff, oversee patient services, ensure compliance with ACHC and Medicare rules, and participate in QAPI activities, aiming to maintain high-quality patient care in home hospice settings.

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Must be a Registered Nurse licensed by the State of OHIO.
• Minimum of 2 years nursing experience in home hospice or skilled home care; hospice preferred.
• At least one year of supervisory experience.
• Ability to market and deal tactfully with customers and the community.
• Knowledge of business management, governmental regulations and ACHC standards.
• Effective communication skills, both verbal and written.
• Self-confidence with a positive attitude and commitment to goals.

Responsibilities

• Coordinate and oversee all direct and indirect patient services by clinical staff.
• Provide supervision and guidance to clinical staff to maintain service quality.
• Perform chart audits.
• Gather data for QAPI and participate in QAPI activities.
• Evaluate staff during probation and annually; provide teaching to enhance nursing staff.
• Assess patient needs and home care appropriateness by reviewing intake forms.
• Oversee maintenance of patient clinical records and regulatory compliance.
• Assist with orientation of new staff.
• See patients in the home setting as needed.
• Participate in audits, IDG meetings and QAPI.
• Maintain adequate medical supplies.
• Responsible for orientation and ongoing in-services.
• Ensure Medicare, State and ACHC regulations are followed by clinical staff.
• Pursue self-development through education, workshops, conferences, and reading.
