Job summary

Buckeye Hospice and Palliative Care Services is seeking a Clinical Services Coordinator/Director of Clinical Services to lead the hospice clinical programs and ensure high-quality patient care. Qualifications

• Must be a graduate of an accredited School of Nursing.
• Must be an Ohio-licensed RN with ability to administer meds and perform nursing procedures.
• Minimum 2 years nursing experience in home hospice or skilled home care.
• At least 1 year of supervisory experience.
• Ability to market and deal tactfully with customers and the community.
• Knowledge of business management, governmental regulations and ACHC standards.
• Excellent verbal and written communication skills.
• Self-confident with a positive attitude and commitment to goal achievement.

Responsibilities

• Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
• Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.
• Perform chart audits.
• Gather data for QAPI and participates in QAPI activities.
• Appraise clinical staff performance during probation and annually, provides teaching and counseling to enhance delivery and professional development.
• Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form.
• Oversees maintenance of patient clinical records and ensures compliance with laws and ACHC standards.
• Assist with orientation of new staff.
• See patients in the home setting as needed.
• Participate in audits, IDG meetings and QAPI.
• Maintains adequate and appropriate Medical Supplies.
• Responsible for orientation of new personnel as well as in-services for current staff.
• Responsible for ensuring Medicare, State and ACHC regulations are followed by all clinical staff.
• Pursue self-development through formal education, workshops and conferences.
