Job summary

Socket.dev is seeking a HCS Care Coordinator to manage care plans for individuals in the HCS Program. You will coordinate services, obtain consents, monitor ISP/IPC, and communicate with families and LARs to ensure quality of care.

The role requires a Bachelor's degree in a related field and at least three years of relevant experience, including supervisory exposure, with strong computer skills and compliance knowledge.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Human Services, or related field.
• Three years’ experience in developmental disabilities, including supervisory experience.
• Proficiency with Windows 7 and Microsoft Office 2010; ability to learn additional software.

Responsibilities

• Coordinate and monitor development and delivery of HCS Program services.
• Ensure care plans align with team recommendations and individual goals.
• Obtain and maintain necessary consents and file documentation.
• Manage IPC/ISP coordination and ensure current records in the individual’s file.
• Coordinate vendors and supervise service delivery.
• Maintain contact with individuals, families, and representatives; inform changes in ISP.
• Provide copies of Individual Rights, Grievance Procedures, and contact information.
• Coordinate pre-discharge and discharge plans; arrange transportation as needed.
• Monitor and ensure services meet HCS guidelines and regulatory requirements.
