Job summary

Greater Hall Chamber of Commerce is seeking a skilled health program caseworker to provide targeted services to clients with health needs across multiple counties. You will coordinate care plans, monitor progress, and collaborate with agencies to secure needed services.

Requires a bachelor’s degree in social work or related field (or RN license), two years of relevant experience, strong communication, and travel willingness to serve a large geographic area.

Qualifications

• Bachelor's degree in social work, psychology, sociology or related field or Registered Professional Nurse currently licensed to practice in Georgia.
• Two years of experience in human services or health related field.
• Valid State Driver’s License.

Responsibilities

• Brokers the CCSP Services and implements the care plan.
• Meets the standard of promptness for 10-day phone calls for new admissions.
• Meets the standard of promptness for the 60-day comprehensive care plan in consultation with the client, client’s family, and service providers.
• Arrange for non-CCSP community-based services as needed by client.
• Monitors service delivery to individual clients to assure services are being provided as appropriate and effectively meets the client’s needs.
• Continuously reviews, monitors, and updates the comprehensive care plan.
• Document case activity and service information.
• Participates in case conferences with the RN to discuss the plan of care as needed.
• Communicates and coordinates with all agencies providing direct services to the clients.
• Approves/disapproves provider’s request for increased services.
• Meets the standard of promptness for quarterly CCP reviews.
• Reports suspected abuse to LTCO and ORS if client lives in a PHC.
• Communicates with DFACS regarding MAO/PMAO eligibility.
• Maintain Knowledge of the provider services for each CCSP Service.
• Assist clients with appeals and attends hearings.
• Attends network meetings, trainings and other meetings coordinated by the AAA/ADRC, Health programs Director, or DCH.
• Adheres to all HIPAA guidelines at all times to ensure Privacy of client records.
• Other duties as assigned.
