Job summary

Kandiyohi County is seeking an HCBS Waiver Case Manager to coordinate Home and Community-Based Services for CAC, CADI, BI, and DD waivers. You will assess needs, authorize services, develop person-centered support plans, and monitor outcomes to promote health, safety, independence, and quality of life.

The role requires a Bachelor's degree in a related field, drivers license, and may require MNChoice certification.

Qualifications

Bachelor’s degree in Social Work, Nursing, Public Health, Human Services, or a closely related field required.
Certified Public Health Nurse (PHN), Registered Nurse (RN), or Social Worker preferred.
Two years of related experience preferred.
MNChoice Certification may be required based on agency needs.
Valid driver’s license and reliable transportation required.
Must pass a criminal background check and meet DHS requirements for vulnerable populations.

Responsibilities

Provide case management services to CAC, CADI, BI, and DD waivers for health, safety and community integration.
Conduct initial assessments, reassessments, monitoring visits, and service reviews.
Complete MNChoice assessments and other required evaluations.
Develop, implement and monitor person-centered support plans.
Authorize waiver services and coordinate with providers, families, and professionals.
Monitor service delivery and ensure compliance with plans and regulations.
Advocate for individuals to access appropriate services and supports.
