Job summary

Missouri Department of Health and Senior Services is seeking a dedicated HCBS program professional to assess and coordinate services for seniors and adults with disabilities. The role focuses on enabling clients to live in the least restrictive environment while coordinating home-based supports.

The position requires a Bachelor’s degree or higher, Missouri licensure for RN/LPN if applicable, and strong communication skills.

Qualifications

• Bachelor's degree or higher from an accredited college or university (relevant experience may substitute for education).
• RN & LPN must be licensed and in good standing in Missouri.
• High level communication and interviewing skills.
• Experience with elderly, disabled, or vulnerable populations preferred.

Responsibilities

• Conduct initial assessments/reassessments to support least restrictive living options.
• Coordinate HCBS services by contacting participants and providers for service authorizations.
• Develop and implement care plans meeting participant needs.
• Travel within local multi-county area for home visits and trainings; minimal overnight travel.
• Ensure program integrity, operations and success.
• Represent DSDS in administrative hearings.
