Job summary

The Missouri Department of Health and Senior Services is seeking a qualified professional to perform assessments and coordinate Home and Community Based Services for seniors and adults with disabilities in the Steelville area. You will develop and implement care plans, contact participants and providers, and ensure services meet participant needs.

Travel within a multi-county area is required, utilizing your own vehicle.

Qualifications

High-level communication and interviewing abilities.
Ability to work independently and take initiative within a team.
Detail-oriented with strong documentation skills.
Experience with adults who are elderly or have disabilities is preferred.

Responsibilities

Conduct initial assessments and reassessments to support least restrictive living environments.
Coordinate Home and Community Based Services (HCBS) by contacting participants and providers to authorize services.
Develop and implement care plans addressing unmet needs with participant input.
Travel within a multi-county area to conduct home visits, hearings, and trainings.
Maintain program integrity, operations, and accountability for DSDS.
Represent DSDS in administrative hearings.
