Job summary

Imagine the Possibilities, Inc. in Winterset, IA seeks an HCBS Programs Lead to supervise and coordinate services in home and community-based settings. You will monitor services, develop strategies, and lead a team to empower people to achieve their personal goals.

The role is in-office with a Monday–Friday schedule and an on-call rotation. Benefits include PTO, 401k, comprehensive insurance, and advancement opportunities.

Qualifications

• Must be 18 years of age or older.
• Driver's license and reliable transportation required.
• Educational requirement: High School Diploma/GED.
• Able to work in the U.S. without sponsorship.
• Physical ability to lift up to 35 pounds and assist as needed.

Responsibilities

• Lead and develop a strong team culture with regular meetings and on-call participation.
• Monitor resources like vehicle usage, budgets, payments, meds, and scheduling.
• Coordinate person-centered services with CBCM and IDT; develop plans and assess needs.
• Serve as primary liaison to individuals, families, guardians, and case managers; document changes.
• Maintain compliance, document accurately, and advocate for independence and dignity.
