Job summary

Cambridge Nonprofit Coalition in Somerville, MA, seeks a Director of Home and Community-Based Services to lead HCBS programs, ensuring quality, compliance, and financial stewardship across Home Care, Community Nursing, Housing Supports, and transitions of care.

Reporting to the COO, the Director will translate strategic priorities into actionable plans, develop performance dashboards, coach managers, and foster a culture of accountability, collaboration, and person-centered services.

Qualifications

• Five years of progressive leadership experience in Long-Term Services and Supports (LTSS) or HCBS.
• Experience in aging, disability, home care, care management, or managed care.
• Master's degree preferred; licensure/credentials in nursing or social work highly valued.
• Experience in a unionized environment preferred.
• Proficiency with Microsoft Office, SharePoint, and data dashboards.

Responsibilities

• Provide day-to-day operational leadership for HCBS programs, ensuring effective service delivery, regulatory compliance, financial stewardship, and achievement of performance expectations.
• Translate organizational priorities into clear operational goals, workflows, timelines, and accountability measures.
• Develop and monitor HCBS improvement plans, ensuring initiatives have clear ownership, timelines, and expected outcomes.
• Partner with Directors and department leaders to coordinate implementation efforts and resolve barriers to successful execution.
• Lead responses to regulatory reviews, audits, monitoring visits, and corrective action plans, ensuring timely implementation and sustained compliance.
• Monitor HCBS operational performance through dashboards, reports, and performance measures; coach managers accordingly.
