Job summary

Imagine the Possibilities, Inc. is seeking an HCBS Programmer for an in-office, Monday–Friday role in Iowa. You will lead a team, monitor services, and coordinate person-centered plans in a Home and Community-Based Services setting.

You’ll oversee budgets, vehicles, medication check-ins, and scheduling while ensuring compliant documentation and advocacy for individuals’ independence and well-being. An on-call rotation is required.

Responsibilities

• Lead and maintain a strong team culture with monthly meetings and on-call participation.
• Oversee resources like vehicle usage, budgets, and appointment scheduling to ensure smooth service delivery.
• Coordinate person-centered services with CBCM and IDT, develop plans, and maintain documentation and authorizations.
• Serve as primary liaison to individuals, families, guardians, and case managers; document changes and support community partnerships.
• Maintain compliance with laws and policies; ensure accurate incident reporting and advocate for independence and dignity.
