Gateway District Health Department in Kentucky seeks a HANDS home visitor to support first-time and multi-gestation families. You will provide regular visits, facilitate health service referrals, and maintain confidential case records while supporting child development and parenting goals.
The role requires on-site work within the district with occasional travel; training, supervision, and productivity standards are part of the employment terms. 18+ and a driver’s license are required.
Employee is a trained home visitor (Para-Professional) for the HANDS program in the state of Kentucky. Credentials will be kept on file and updated on a yearly basis or as changes arise. The pay scale range will be $13.50 - $20.07 per hour/75 hours bi-weekly. A cell phone reimbursement of up to $75 per month will also be provided. The workstation will be Morgan County Health Department with travel outside Morgan County determined by District caseload needs. Travel will be reimbursed at the approved state rate. EMPLOYEES IN THIS CLASS WILL BE REQUIRED TO OBTAIN CREDENTIALS OUTLINED BELOW WITHIN A REASONABLE TIME, OR IF EMPLOYEES POSSESS SUCH CREDENTIALS AT THE TIME OF HIRE THEY WILL BE REQUIRED TO MAINTAIN THOSE CREDENTIALS SO LONG AS THEY ARE EMPLOYED IN THIS CAPACITY. On the job training requirements needed within 6 months of hire in order to advance: HANDS home visitor core training (Must be completed before family contact can occur), HANDS wrap-around required training (Specific topical areas checklist), CPR Certification, Working knowledge of geographic area and or ability to locate participants and community resources with use of maps, contacts, and directions provided.

Conduct initial home visit following Parent Visitor assessment with first time parent/s and or families to establish rapport and planning for providing services as qualified by the provider. Provide regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant/s to assist first time parent/s and or families with child development, parenting skills, health services and other needed resources identified at assessment or during visits. Facilitate families in securing of prenatal/child health services, referrals, and appointments with other supportive agencies. Document case records, home visit logs accurately on each contact, progress, service need and completes reports as required. Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers. Confer with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.

The Kentucky Department for Public Health (DPH) is responsible for developing and operating state public health programs and activities for the citizens of Kentucky. The mission of DPH is to improve the health and safety of people in Kentucky through prevention, promotion and protection. The department oversees programs designed to improve the lives of citizens and visitors through prevention of negative health outcomes, promotion of healthy lifestyles and protection from diseases, injury and environmental health impacts.
