Job summary

LiveWell Homecare Agency in Georgia seeks a Certified Hand Therapist (CHT), Occupational Therapist to provide rehabilitation services across Northwest Metro Atlanta. You will assess, plan, and implement hand and upper-extremity therapy to improve function.

Responsibilities include collaborating with physicians, educating patients, documenting care, and ensuring regulatory compliance while delivering ethical, patient-centered care.

Qualifications

• Master's or Bachelor's degree in Occupational Therapy.
• Valid OT license.
• BLS certification.

Responsibilities

• Assessing, planning, and implementing individualized treatment plans to improve patients' hand and upper-extremity function.
• Revising care plans with patient and family input.
• Educating patients and providing discharge instructions and follow-up care.
• Collaborating with physicians to establish and evaluate treatment programs.
• Documenting evaluations, treatment sessions, and discharge plans according to departmental standards.
• Ensuring compliance with regulatory standards and maintaining patient confidentiality.
• Participating in staff meetings, ongoing education, and team activities.
