Job summary

Mosaic Beatrice is seeking a Habilitative Coordinator/Case Manager to oversee a diverse caseload of up to 22 individuals, delivering person-centered supports.

This full-time salaried role requires coordinating assessments, leading annual reviews, collaborating with families and guardians, coaching staff, and ensuring high-quality, compliant care in our community-based program. A Bachelor's degree and 3+ years with ID/DD are preferred.

Qualifications

• Education: REQUIRED - Bachelor's Degree in Psychology, Special Education, or a closely related field.
• Experience: At least 3 years of hands-on experience working with individuals with intellectual/developmental disabilities.
• Expertise: Deep knowledge of habilitative services, Intermediate Care Facilities state regulations, and behavior intervention techniques.
• Certifications: QIDP certification (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification required (if you do not have then required to obtain after hire).
• Core Competencies: Exceptional critical thinking, strong problem-solving ability, excellent written communication, and structured time-management skills.

Responsibilities

• Oversee a diverse caseload of up to 22 individuals, creating dynamic, individual-centered programs.
• Coordinate and present key assessments (CFA, POM, SIS), lead annual review meetings, and present before the Human Rights Committee.
• Build strong partnerships with Home Supervisors, families, guardians, and Service Coordinators to drive admission dates and maintain regulatory compliance.
• Train direct care staff in behavior modification techniques and help develop innovative service methodologies.
• Monitor the quality of care provided and maintain clear, professional documentation.
