Job summary

Mosaic Beatrice invites a dedicated professional to join as a Person Centered Plan Case Manager. You will oversee a caseload of up to 22 individuals, developing individualized programs to support growth and independence in a Beatrice community setting.

You will lead annual meetings and assessments (CFA, POM, SIS) and present before the Human Rights Committee, while coordinating with supervisors, families, guardians, and service coordinators to drive admissions and ensure regulatory compliance.

Qualifications

• Bachelor's degree in psychology, special education, or related field.
• At least 3 years of hands-on experience with individuals with intellectual/developmental disabilities.
• QIDP certification (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification required or obtained after hire.

Responsibilities

• Oversee a caseload of up to 22 individuals with individual-centered programs.
• Lead annual meetings and assessments (CFA, POM, SIS) and present before the Human Rights Committee.
• Collaborate with Home Supervisors, families, guardians, and Service Coordinators to drive admission dates and ensure regulatory compliance.
• Coach direct care staff in behavior modification techniques and help develop innovative service methodologies.
• Maintain high standards by monitoring quality of care and keeping clear, professional documentation.
