Job summary

BAYADA Home Health Care seeks a Habilitation Manager (Qualified Professional) for the State College, PA office. The role involves coordinating care, supporting person-centered plans, and advocating for clients in the Habilitation program.

Ideal candidate has a Bachelor degree and at least 1 year post-degree experience with I/DD populations, with strong communication skills. Regular hours and supportive team culture are offered.

Qualifications

• Bachelor degree is required.
• Must be a Qualified Professional - minimum of 1 year of post degree experience with I/DD population.
• Prior supervisory experience is a plus.
• Demonstrated record of taking on increased responsibility.

Responsibilities

• Act as internal case manager and advocate for clients in the Habilitation program.
• Participate in Person-Centered Planning processes.
• Collaborate with client, family, Case Managers, Hab Techs and other teams to implement goals in the Individual Support Plan.
• Make regular client visits to evaluate progress and ensure services align with the plan.
• Evaluate progress of goals identified in the plan.
• Maintain ongoing communication with Case Managers and Care Coordinators.
• Supervise Habilitation Technicians per policy and state rules.
• Participate in client-specific orientations as directed.
