Job summary

BAYADA Home Health Care in Lebanon, MO has an immediate opening for a Habilitation Manager (Qualified Professional). This leadership role acts as an internal case manager and advocate within the Habilitation program, ensuring client goals are met through Person-Centered Planning and collaborative care.

The successful candidate will have a Bachelor degree, at least 2 years of post-degree experience with the I/DD population, and demonstrated supervisory ability.

Qualifications

• Bachelor degree is required.
• Must be a Qualified Professional with a minimum of 2 years of post-degree experience with the I/DD population.
• Supervisory experience is a plus.
• Demonstrated record of taking on increased responsibility.

Responsibilities

• Act as internal case manager and advocate for clients in the Habilitation program.
• Participate in Person-Centered Planning processes.
• Collaborate with client, family, Case Managers, Hab Techs and teams to implement goals.
• Make regular client visits to evaluate progress and ensure services align with plans.
• Maintain ongoing communication with Case Managers/Care Coordinators.
• Supervise Habilitation Technicians according to policy and state regulations.
• Participate in client-specific orientations as directed by Clinical Manager.
