BAYADA Home Health Care in Wilkes-Barre, PA is seeking an I/DD Program Manager to advocate for clients receiving habilitation services and lead a multidisciplinary team. You will align services with ISP goals, oversee care plans, and drive client progress through regular visits and coordinated planning.

The role requires leadership experience, a Bachelor's degree, and a passion for empowering individuals with I/DD to achieve greater independence and improved quality of life.

Qualifications

• Bachelor's degree required.
• Experience working with the I/DD population required.
• Prior supervisory or leadership experience required.
• Strong communication, organization, and relationship-building skills.
• Ability to collaborate with clients, families, and interdisciplinary teams.

Responsibilities

• Serve as an internal case manager and advocate for clients receiving habilitation services.
• Participate in Person-Centered Planning and support the development of individualized goals.
• Collaborate with clients, families, Case Managers, Habilitation Technicians, and interdisciplinary teams to implement and evaluate care plans.
• Conduct regular client visits to assess progress and ensure ISP alignment and program requirements.
• Monitor and document progress toward individual goals and outcomes.
• Maintain strong communication with Case Managers, Care Coordinators, families, and other stakeholders.
• Supervise, coach, and support Habilitation Technicians in line with policies and state regulations.
• Participate in client-specific orientations and training.
• Support client intake activities and travel locally as needed.
