Job summary

BAYADA Home Health Care in State College, PA has an immediate opening for a Habilitation Manager (Qualified Professional). The role partners with the Clinical Manager to advocate for clients in the Habilitation program and to support Person-Centered Planning.

You will supervise Habilitation Technicians, coordinate with Case Managers and care teams, visit clients, monitor service delivery, and contribute to achieving client goals identified in the Individual Support Plan.

Qualifications

• Bachelor degree is required.
• Must be a Qualified Professional with 1+ year post-degree experience in I/DD.
• Supervisor experience is a plus.
• Demonstrated record of increasing responsibility.
• Ambition to grow beyond the current position.
• Excellent communication and interpersonal skills.

Responsibilities

• Act as internal case manager and advocate for clients in the Habilitation program.
• Participate in Person-Centered Planning processes.
• Collaborate with client, family, Case Managers, Hab Techs and other teams to implement goals.
• Make regular client visits to evaluate progress and service delivery.
• Evaluate progress of goals in the Person Centered/Individual Support Plan.
• Maintain ongoing communication with Case Managers/Care Coordinators.
• Supervise Habilitation Technicians per policy and state regulations.
• Participate in client-specific orientations as directed by the Clinical Manager.
