Job summary

BAYADA Home Health Care in Philadelphia, PA is seeking a Senior Habilitation Manager to oversee Habilitation Technicians and ensure services align with client goals.

You will collaborate with families, case managers, and interdisciplinary teams, provide supervision and coaching, and contribute to program quality and person-centered planning. A Bachelor's degree and 2+ years in I/DD required.

Qualifications

• Bachelor's degree required.
• Minimum of 2 years of post-degree experience working with individuals with I/DD.

Responsibilities

• Serve as an internal case manager and advocate for clients receiving Habilitation services.
• Participate in Person-Centered Planning and ISP processes.
• Collaborate with clients, families, Case Managers, Habilitation Technicians, and other care team members to identify and meet goals.
• Conduct regular client visits to assess progress and ensure services align with plans.
• Provide supervision, coaching, and support to Habilitation Technicians.
• Support orientations and training as directed by Clinical Manager.
