Qualifications

• Bachelor degree is required.
• Must be a Qualified Professional with at least 2 years post-degree experience with I/DD population.
• Supervisory experience is a plus.
• Demonstrated record of taking on increased responsibility.
• Strong communication and interpersonal skills.

Responsibilities

• Act as internal case manager and advocate for clients in the Habilitation program.
• Participate in Person-Centered Planning processes.
• Collaborate with the client, family, Case Managers, Hab Techs and others to implement and evaluate care needs and goals.
• Make regular client visits to evaluate progress and ensure services align with the plan and service definitions.
• Evaluate progress of goals identified in the plan.
• Maintain ongoing communication with Case Managers and Care Coordinators.
• Supervise Habilitation Technicians according to policy and state regulations.
• Participate in client orientations as directed by the Clinical Manager.
