Job summary

BAYADA Home Health Care in Allentown, PA is seeking a Program Specialist to act as an internal case manager and advocate for clients in the Habilitation program. The role involves coordinating with families, case managers, Hab Techs and other teams to meet client goals.

Responsibilities include conducting client visits, monitoring service delivery, and supervising habilitation staff while participating in Person-Centered Planning and client orientations.

Qualifications

• Bachelor degree is required.
• Experience with I/DD population required.
• Prior supervisory experience is a plus.
• Demonstrated record of taking on increased responsibility.
• Ambition to grow and advance.

Responsibilities

• Collaborate with Clinical Manager as internal case manager and advocate for clients in the Habilitation program.
• Participate in Person-Centered Planning processes.
• Coordinate with client, family, case managers, Hab Techs on care goals.
• Perform regular client visits to monitor progress and service delivery.
• Evaluate progress of goals in the Person Centered/Individual Support Plan.
• Maintain ongoing communication with case managers and care coordinators.
• Supervise Habilitation Technicians as required by policy and state regulations.
• Assist with client orientations as directed by Clinical Manager.
• Travel locally for client intake conversations.
