Job summary

BAYADA Home Health Care in Rolla, MO is seeking a Habilitation Manager (Qualified Professional) to lead client care planning and service delivery within our Habilitation program. You will work closely with the Clinical Manager, families, and care teams to implement plans and monitor progress.

The role involves conducting client visits, coordinating with case managers, supervising habilitation technicians, and leading orientations to ensure quality outcomes aligned with the Person

Qualifications

• Bachelor degree is required.
• Must be a Qualified Professional with minimum 2 years post-degree experience with I/DD population.
• Prior supervisory experience is a plus.

Responsibilities

• Act as internal case manager and advocate for clients in the Habilitation program.
• Participate in Person-Centered Planning processes.
• Collaborate with client, family, Case Managers, Hab Techs and teams to implement and evaluate care plans.
• Make regular client visits to evaluate progress and ensure services align with the plan.
• Maintain ongoing communication with Case Managers/Care Coordinators.
• Supervise Habilitation Technicians according to policy and state regulations.
• Participate in client-specific orientations as directed by Clinical Manager.
