Job summary

Alpert Jewish Family Service in West Palm Beach, FL seeks a Guardianship Case Manager to coordinate intake, evaluation, and service provision for Wards appointed by courts or voluntarily served by the agency.

The role involves conducting bio-psychosocial assessments, developing care plans, and maintaining detailed case records to meet Florida requirements. Strong communication and teamwork with the Guardianship team are essential.

Qualifications

• Graduate degree from an accredited School of Social Work.
• 3–5 years' experience in a similar role.
• Experience with geriatric and special populations.
• Knowledge of MS Office Suite and ERM systems.
• Valid Florida driver's license and good driving record.
• Strong communication and interpersonal skills.

Responsibilities

• Takes part in regular and ad hoc meetings to assess Ward needs and plan services.
• Reviews and communicates with Guardianship team about Ward information and services.
• Receives bills, processes checks, and handles vendor communications.
• Performs bio-psychosocial assessments in client settings as appropriate.
• Develops service plans based on assessments to meet client needs.
• Prepares visits to Ward and family to gather information for court will/needs.
• Intervenes to maintain functional independence for clients.
• Provides outreach and information sharing among professionals.
• Maintains case records to meet quality standards and Florida law.
