EverTrue is seeking a Bereavement Coordinator to provide bereavement and social services support for patients and their families in our hospice program. You will oversee the bereavement services, educate families, and respond to crisis needs.

You will contribute to group memorials, coordinate with the interdisciplinary team, and help plan community programming while documenting care in the EMR system.

Qualifications

• Bachelor’s Degree required.
• Medicare hospice experience preferred.
• Strong communication and confidentiality skills; ability to work with diverse populations.

Responsibilities

• Oversee the bereavement program including mailings, calls, and services.
• Provide bereavement education and support for patients and families.
• Respond to emergent bereavement counseling needs.
• Coordinate group memorial services and community programming.
