Job summary

Heart to Heart Hospice in Texas seeks a Bereavement Coordinator to lead bereavement support for families and coordinate care plans with the interdisciplinary team. You will connect families with community resources and maintain thorough documentation.

The role requires counseling experience, strong communication and public relations skills, and reliable transportation with a valid driver's license. This is a full-time, on-site position with growth opportunities.

Qualifications

• Bereavement and grief counseling experience, preferably in hospice/palliative care.
• Working knowledge of community resources.
• Demonstrates strong communication, interpersonal, negotiation and public relations skills.
• Experience with EMR software is a plus.
• Valid driver's license with reliable transportation and current auto liability insurance.

Responsibilities

• Assess hospice family to identify bereavement needs and refer to community resources.
• Maintain knowledge of public and private eligibility standards.
• Collaborate with interdisciplinary team to coordinate bereavement care plans and interventions.
• Provide continued bereavement support to hospice families with documentation.
• Participate in QAPI and train/monitor volunteers supporting the bereavement program.
• Ensure timely submission of all documentation andCompliance with laws and policies.

What You Must Have:

• High School Graduate, Degree/License in counseling preferred
• Experience in bereavement and grief counseling, preferably in hospice and palliative care setting; grief support counseling
• Working knowledge of community resources
• Demonstrates good communication, interpersonal, negotiation, and public relations skills
• Experience with EMR software is a plus
• Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance
