Gilead Community Services, Inc. in Middletown, CT, is seeking a Clinical Case Manager for the adult supervised apartment program.
The role runs Sunday–Thursday, 7:00 AM–3:00 PM (40 hours weekly) and delivers case management, advocacy, education, and supportive services to women with complex mental health and substance use histories. The position requires a Bachelor’s degree in a human services field and one year of clinical experience; a Master’s degree is preferred.

The Clinical Case Manager for Gilead’s Middletown area adult supervised apartment program works Sunday - Thursday 7:00 AM - 3:00 PM (40 hours per week) providing case management, advocacy, educational, and supportive services in a program serving women with major mental illness, co-occurring substance use disorders and histories of significant trauma. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.
This is a union position and pays $20.38 per hour dependent on level of education.
