Job summary

Providence PACE is seeking a Nurse Practitioner or Physician Assistant for a full-time role in Portland, Oregon, joining a multidisciplinary, multi-center program serving older adults. The position involves care across seven centers and in various community-based settings, with opportunities for continuity coverage and collaboration with a broad care team.

Qualifications

• One year of post-graduate clinical experience working with a geriatric population preferred
• Training and/or experience in palliative care and/or hospice valuable

Responsibilities

• Float across seven Providence PACE centers in the Portland metro area to provide care
• Deliver care in a variety of settings including community-based visits and outpatient clinics
• Collaborate closely with physicians, nurse practitioners, nurse care managers, social workers, pharmacists, and other team members
• Support continuity coverage by temporarily assuming a patient panel for up to six months
• Commit to a team-based model focused on comprehensive whole-person geriatric care
